- What Is EDWP - The Elderly and Disabled Waiver Program (formerly known as the Community Care Services Program - CCSP) provides in-home and community-based services for frail, elderly, and disabled Georgians. These services can take place either in the person's home or within the community itself.
- Our Services Through EDWP - 1. Skilled Nursing: Various services performed by a licensed nurse that includes wound care, medication management, rehabilitation, pain management, and patient education. 2. Personal Care Assistance: Assistance with daily activities such as bathing, dressing, and grooming. 3. Respite Care for Family Caregivers: Temporary relief for caregivers.
- How to Apply - 1. Eligibility: To qualify, you must be eligible for Medicaid, at least 65 years old (or meet certain disability requirements), and choose home or community services over nursing home care. 2. Application Process: Contact your local **Area Agency on Aging (AAA)** by calling the toll-free number: **866-552-4464**. An AAA staff member will screen you for eligibility and urgency, placing you on the waiting list. 3. Next Steps: Once your name reaches the top of the waiting list, an in-home assessment will determine your care needs. Our nurse or care coordinator will work with you and your caregiver to develop a personalized care plan.
